The stored image is square, but every place it appears it is masked to a circle. The corners are always cut. Centre the subject and leave breathing room around it — a face cropped tight in a square will lose its chin and forehead once the mask is applied.
Upload larger than 320
Instagram downsizes whatever you give it, and its own scaler produces a cleaner result than a small image stretched. Feeding it something around 1080×1080 and letting it shrink usually looks sharper than uploading exactly 320×320.
